Community Vaccination Clinic Open In Highland Park
Online self-registration for the April 8 community vaccination clinic at the Highland Park Country Club is available. Registration is open to anyone who is currently eligible under state guidelines (Phase 1a, 1b, 1b+).

Appointments are available on a first-come, first-served basis. You will receive an email confirmation from Jewel with pre-authorization documents that should be completed before your appointment.
Vaccination Location on April 8th by appointment only: Highland Park Country Club 1201 Park Avenue West, Highland Park, IL 60035
At your appointment: •
- Plan to arrive ten minutes earlier. •
- Bring your ID confirming your current residence in Highland Park, completed authorization forms, and your insurance card. •
- The vaccine will be delivered in one dose. •
- You will not have to pay for the vaccine. •
- After you receive your vaccination, you will need to wait 15 minutes for observation. Important things to note: •
